Your AI model says it follows your company's policies. How do you know? Because it told you so. We ran an audit on a system that was auto-generating compliance rules from corporate policy docs. 76% of those rules contained constraints that didn't exist in the source text. Plausible. Well-written. Completely fabricated. Nobody caught it because nobody was looking. That's the problem. We deployed 15 specialist raters. 3 policy frameworks.
