🚀 Announcing the launch of a new Rare Horizons resource designed to help improve the transition from paediatric to adult care for adolescents and young adults living with rare diseases. This first-of-its-kind collaboration brings together youth advocates, patient organisations, clinicians, nurse specialists and psychologists to address one of the most important, and often challenging stages of the patient journey.
