Advancing the future of palliative care—together 👍 As SLH continues its journey to become a palliative-care-ready hospital, our multidisciplinary team of doctors, nurses, and allied health and pharmacy professionals attended the Singapore Palliative Care Conference (SPCC) to learn, share, and advance best practices in end-of-life care. We are proud to have two projects featured as conference abstracts, contributing to the growing body of palliative care knowledge: 🔹 The Biopsychosocial-Spiritual Model in End-of-Life Care for Persons with Intellectual Disability (PWID) — exploring more holistic palliative care for persons with intellectual disability through a team’s experience caring for a patient with Down syndrome. 🔹 Achieving a Good Death for the Dying in SLH — demonstrating how structured care pathways and interdisciplinary education improved end-of-life outcomes, guided by standards shaped through literature reviews, national palliative care benchmarks, and team discussions involving a caregiver. Our Medical Director, Dr Andrew Samson, also joined a panel discussion on “Palliative Care as Value Based Primary Care”, sharing how SLH’s palliative ecosystem enables timely, person-centred, and goal-concordant care. By integrating palliative principles earlier in primary care, patients benefit from better continuity and support aligned with their values and priorities throughout their illness.
