This #NursingWeek, Toronto Star highlights the compassionate work of nurses like Jia Ong and Althea Moncrieffe, who are part of SHN’s Nurse-Led Outreach Team (NLOT) that brings hospital-level expertise directly to long-term care (LTC) home residents. NLOT nurses provide essential services right where residents live, such as IV therapy, wound care, and G-tube management—helping them stay in familiar surroundings and avoid distressing hospital visits. The Star interviewed Jia, nurse practitioner, and Althea, registered nurse, about the impact of their work and how community-centered care is essential.
