From Hospital Universitario de Basurto, Bilbao, España, an innovative Hybrid Percutaneous Solution using a suture-based device for a complex Interatrial Defect The Revista Argentina de Cardioangiología Intervencionista (RACI) proudly highlights this clinical image case presentation by Uxue Millet Oyarzabal et al., describing a hybrid percutaneous approach for the treatment of concomitant patent foramen ovale (PFO) and atrial septal defect (ASD). This case illustrates the importance of individualized structural heart intervention in anatomically complex interatrial septa. The combined use of an Amplatzer™ occluder for ASD closure as usual, together with the NobleStitch™ EL system for PFO closure (using a percutaneous suture), represents a highly innovative strategy aimed at minimizing intracardiac material while achieving effective defect closure. The report emphasizes: - Tailored therapy based on septal anatomy - Advanced imaging guidance with transesophageal echocardiography - Hybrid structural intervention techniques - Reduction of intracardiac device burden in selected patients. The NobleStitch EL™ is a minimally invasive, suture-mediated system used to close a Patent Foramen Ovale (PFO). Unlike traditional PFO closure, which leaves a permanent metallic umbrella-like device in the heart, this "deviceless" technique uses specialized stitches to suture the heart wall together. How does the procedure work? The NobleStitch technique is performed in three steps: Septum Secundum: A suture is placed through the thicker wall (septum secundum) of the heart. Septum Primum: A second suture is placed through the thinner flap (septum primum). KwiKnot Fixation: A specialized catheter device (KwiKnot) is used to draw the two flaps tightly together, locking the sutures in place and closing off the PFO.
