Expanding EVAR to Challenging Anatomy The Revista Argentina de Cardioangiología Intervencionista (RACI) proudly highlights this remarkable case report by Nunes Da Cruz et al., describing the successful endovascular repair of an infrarenal abdominal aortic aneurysm with an extremely short neck using the ALTO® Endologix endograft. This case illustrates how modern polymer-based sealing technology can expand the boundaries of EVAR in patients traditionally considered unsuitable for conventional endovascular repair. In an 83-year-old high surgical risk patient with a 7 mm infrarenal neck, the use of the ALTO® system achieved: ✔️ Effective proximal sealing ✔️ Preservation of renal and hypogastric flow ✔️ Absence of endoleaks ✔️ Successful exclusion of the aneurysm without adjunctive complex techniques The report demonstrates the importance of individualized device selection, meticulous CT planning, and advanced endovascular technologies in managing hostile neck anatomy.
