How can we optimise gentamicin dosing in neonates and infants? 🤔 Gentamicin is an important treatment of serious neonatal infections, yet rapid developmental changes in renal function make dosing challenging during childhood. 👶 In this study, Marika de Hoop-Sommen and colleagues used PBPK modelling to simulate current dosing regimens and evaluated both efficacy targets and safety thresholds. While peak concentrations generally met efficacy targets, trough levels in neonates were often elevated, raising concerns about toxicity. Extending the dosing interval improved the benefit–risk balance. For older infants, minor dose adjustments were suggested to optimise exposure.
