🟣 Localized Prostate Cancer: 96.4% Biochemical Control at 2 Years After SBRT A retrospective multicenter study evaluated the real-world outcomes of stereotactic body radiotherapy in 250 men with localized prostate cancer treated across 12 Spanish centers between 2020 and 2023. Patients received 36.25–40 Gy in five alternate-day fractions. At 24 months: ▫️ Biochemical recurrence-free survival: 96.4% ▫️ Radiological progression-free survival: 97.2% ▫️ Overall survival: 95.6% ▫️ Cancer-specific survival: 100% ▫️ Grade ≥2 late genitourinary toxicity: 7.6% ▫️ Grade ≥2 late gastrointestinal toxicity: 1.2% ▫️ Grade ≥2 erectile dysfunction: 14.3% among evaluable patients Biochemical recurrence occurred in 9 of 247 evaluable patients and was limited to those with unfavorable intermediate- or high-risk disease and ISUP grade group ≥3. Diabetes was independently associated with a higher risk of grade ≥2 late genitourinary toxicity, with an adjusted odds ratio of 6.10. These findings provide multicenter real-world evidence of high early biochemical control and low rates of late GU and GI toxicity after prostate SBRT. Longer follow-up is needed to assess the durability of disease control and longer-term toxicity.
