AbbVie and Genmab reported that the Phase 3 EPCORE DLBCL-4 trial hit its primary endpoint. In second-line relapsed/refractory DLBCL, epcoritamab plus lenalidomide cut the risk of progression or death by 60% versus R-GemOx (HR 0.40; p<0.0001) — from a fixed-duration, chemo-free regimen. Our new analysis unpacks what this means for a genuinely hard-to-treat population (post-transplant relapse, CAR-T–ineligible), how the chemo-free design happens to sidestep the very issues that tripped up Roche's STARGLO at the FDA, and what still has to land before a second-line label. When the cutting edge medicine is, by design, hard to scale, are we going to build the system around the breakthrough? or the patient that can't get it?
