One of the most interesting clinical challenges in endodontics is managing cases where instrumentation alone is not enough. During her To The Point Lecture at AAE26, Dr. Jianing He presented a case involving an immature tooth with a wide-open canal and extensive necrotic tissue. In this situation, conventional files provided limited value, making irrigation and debridement the critical components of treatment. Without prior instrumentation, ODNE Clean was activated and the necrotic tissue was rapidly expelled from the canal. Beyond the immediate clinical outcome, the case highlights a broader discussion around the role of fluid dynamics in endodontic cleaning and how clinicians may approach anatomies where traditional instrumentation has inherent limitations.
