Cardiometabolic diseases such as diabetes and hypertension often remain invisible in humanitarian crises, yet they place a significant burden on vulnerable, underserved communities. Since 2022, the Danish Red Cross and its partners have been working across Eastern Africa to strengthen continuity of care by embedding prevention, treatment and self‑care within community and primary healthcare systems.   To date, the partnership has supported people living with noncommunicable diseases across urban informal settlements, refugee camps, refugee settlements and crisis-affected rural communities, helping to reduce complications and improve everyday disease management. As the project enters its next phase, there will be a strong focus on scaling resilient, locally anchored models across Kenya, Somalia and Ethiopia, ensuring that people can manage chronic conditions before, during and after crises.
