Congratulations to Holderness Health, the winners of the Quality Improvement Award at the NHS Excellence Awards 🎉 Their Gold Team approach to palliative & end of life care improved communications between GPs and other agencies to provide the much-needed reassurance for patients, their families, carers and staff. Their work has reduced call waits to under a minute and, improved patient identification, creating a service that is centred on compassion. Their approach has boosted staff morale whilst importantly earning praise from patients too. This project highlights how quality improvement work can transform experiences at the most critical moments.
