During Community Health Improvement Week, we’re proud to recognize the people across our ministry who are making a meaningful difference in the health and well-being of the communities we serve.   From expanding access to care and addressing social determinants of health to building stronger community partnerships, this work happens every day, and it’s powered by passionate, dedicated teams. Across the communities we serve, we see this commitment in action through initiatives like the Safe Sleep Initiative focused on reducing preventable sleep-related infant deaths in Lorain County, Ohio and the Community Medication Assistance Program in Springfield, Ohio. These and many other programs are each tailored to meet the unique needs of the communities we serve.   Our teams’ efforts reflect the shared belief that improving community health takes collaboration, innovation and a deep understanding of local needs. Whether it’s connecting individuals to vital resources, advancing preventive care or supporting whole-person health, our teams are helping create stronger, healthier communities.   To everyone involved in this work, thank you for your commitment, compassion and impact.
