Grayanotoxin Poisoning: The Mad Honey Red Flag Grayanotoxin poisoning is a classic plant-toxin trap: beautiful azaleas and rhododendrons, but potentially dangerous exposures through leaves, flowers, nectar, tea, or “mad honey.” Key red flags:bradycardia, hypotension, dizziness, syncope, and AV block. Mechanism: voltage-gated sodium channels fail to inactivate, leading to persistent depolarization and impaired cardiac conduction. Most cases improve with supportive care, but symptomatic patients need poison center consultation, ECG and blood pressure monitoring, IV fluids, and atropine when clinically indicated.
