🔹 174P | The IKF-t018 AURORA Trial: Cabozantinib in HCC After ICI-Based First-Line Therapy Hepatocellular carcinoma (HCC) remains one of the most lethal cancers worldwide. While sorafenib had been the standard of care in the frontline setting for years, the rise of immune checkpoint inhibitors has changed the treatment landscape. The question remains: what is the role of targeted therapies in patients progressing after ICI-based regimens? The phase II IKF-t018 AURORA trial, led by Prof. Prof. Dr. Arndt Vogel Dr. Daniel Pink, Dr. Ulrich Ehmer, Dr. David T. Waldschmidt, Dr. Matthias Damm, Dr. Thomas J. Ettrich, Dr. Jens U. Marquardt, Prof. Salah-Eddin Al-Batran, Dr. Johannes Klagges, and Dr. Andreas Saborowski, explored cabozantinib in patients with advanced or unresectable HCC who were refractory to any first-line therapy other than sorafenib. Among 22 patients enrolled across 7 sites in Germany, cabozantinib achieved a median overall survival of 12.9 months and progression-free survival of 3.0 months. While objective responses were rare (4.5%), outcomes were consistent with previous studies where cabozantinib was given after sorafenib. Treatment-related adverse events were manageable, with grade ≥3 TRAEs reported in 31.8% of patients, leading to dose modifications or discontinuation in some cases.
