The fundamental defect in all of Risk Analysis is the attempt to quantify risks as events has blinded the community to the need to quantify the causal mechanisms that produce outcomes including objectives. People don’t decide between risk events, they make decisions about the causal mechanisms that produce outcomes. Risk analysis is about analyzing causal mechanisms that produce outcomes much more than analyzing just the outcomes.
