88% of AI pilots never reach production.   Each failure costs up to $8.4M and triggers a measurable loss of stakeholder confidence in future AI investment.  The reason is not the technology. Pilots launch with no failure criteria, no portfolio ceiling, and no single accountable owner.  Without a defined exit condition, every pilot runs until the budget runs out. The structural fix is setting a hard ceiling on concurrent pilots. To start a new one, an existing pilot must ship to production or be terminated.
