A new publication by Tessa Folkertsma, Greetje J. Tack-Blokker, Robert Vodegel, Sjaak Bloem, Maya Schroevers, and Reinhard Bos explores how to personalize supportive care for patients with immune-mediated inflammatory disorders (IMIDs). The study concludes that the Subjective Health Experience (SHE) model provides a practical framework for integrated care delivery, clarifying provider roles, enhancing continuity, and fostering interdisciplinary alignment. The findings indicate significant added value for improving care coordination, which in turn increases patients’ perceived continuity of care.
