A project came to us described as running smoothly. A few weeks later, the client's internal team was in crunch. Not because anything had gone wrong with the work itself but because the review process had never really been designed for the volume it was now handling. One approver. No clear prioritisation. Every asset moving through the same queue regardless of complexity. At a smaller scale it had worked fine. As things grew, it became the bottleneck. We spent some time rebuilding the review structure mid-project. Tiered feedback, async cycles, more regular check-ins between leads. The remaining assets shipped closer to schedule than anyone expected at that point. Process problems don't always look like process problems at first.
