One of the most valuable lessons I continue to learn in quality management is that performance improvement is rarely linear; in fact, it is almost never perfect during initial implementation. Overseeing clinical quality management projects has reinforced just how often seemingly well-designed initiatives evolve from onset through execution. In many instances, performance improvement projects are developed with defined aims, measurable outcomes, and evidence-based strategies, yet implementation often reveals workflow gaps and unanticipated barriers. Fortunately, I have learned that this is not failure. It is data. It is opportunity. It is refinement.
