Increased level of operational failures during well completion phases compared to first 6 months in 2014. Cases logged: Tubing Hanger installation failures, top side installations, NSC. Incidents related to TH installation failures not only introduce increased risk to Health Safety and Environment through several stages of recovery and new installation processes, but also severe operational downtime. Up-to-date Detailed Operational Procedures, equipment verification, 3rd party verification and experience transfer could easily have prevented incidents like this.
