A 65-year-old gentleman presented with a large, bulky, lumen-occluding lesion at the D1–D2 junction, involving 50% of the duodenal lumen and extending to within 1 cm of the ampulla. An en bloc ESD was successfully performed, achieving complete resection of a 7 × 3 cm lesion. Marking was carefully performed using a DualKnife with a side-viewing endoscope, caudally. Duodenal ESD, especially in such close proximity to the ampulla, remains one of the most technically challenging procedures in therapeutic endoscopy. By God's grace, the procedure was completed successfully, potentially sparing the patient from a major Whipple procedure.
