Reference ranges are among the most valuable tools in medicine. They help us determine whether a value is statistically unusual within a defined population. But an important question remains: Do reference ranges also tell us where physiology functions best? Being "within range" and being physiologically thriving may not always be identical states. Two individuals can have laboratory values that fall within the same reference range, yet differ substantially in: • metabolic health • physiological reserve • recovery capacity • resilience under stress • long-term health trajectories Perhaps reference ranges answer one question: "Where does this value fall relative to a population?" Assessing health may require us to ask another: "How well is this physiology functioning?" As healthcare continues to evolve, one of our greatest opportunities may be learning how to measure health with the same precision that we measure abnormality.
