"If it's the same kind of data, it must be consistent." It's one of the most common assumptions we hear about real-world data and it's our first Rich, Wild & Dirty myth. Take Disease-Management-Programmes (DMP): structured treatment programmes for chronic conditions, ~8M patients enrolled in Germany, closely monitored quarter after quarter. About as structured as RWD gets. Yet one and the same parameter, namely "unplanned inpatient treatment for CHD since last documentation”, arrives under four different labels across providers, with answers logged as Ja, ja, true, 1… you name it. Structured? Yes. Standardized? Not even close. That's what makes RWD dirty and is the reason why we harmonize to the meaning, not the label.
