Antimicrobial resistance is often discussed as a future threat. The data suggests otherwise. An estimated 1.14 million deaths each year are directly attributable to bacterial AMR, while the pace of resistance continues to outstrip the development of truly novel antibiotics. Yet one contributor receives less attention than it deserves: diagnostic uncertainty. When the pathogen remains unidentified, treatment often becomes broader, stewardship becomes more challenging, and precision becomes harder to achieve. As AMR continues to evolve, the conversation is increasingly shifting from treatment alone to diagnosis as well. Can healthcare systems improve antimicrobial stewardship without improving diagnostic certainty? Abhimanyuu Nambisan Rohan S SALVI Gitesh Srivastava Shuja A.
