Geisinger is proud to receive the Hospital and Health System Association of Pennsylvania’s (HAP) 2026 Achievement Award, recognizing innovative approaches to quality care and community partnership. “This honor reflects our commitment to addressing not only medical needs, but the social factors that shape health,” said Allison Hess, vice president of population health and quality. “By partnering with community organizations and using data to guide decisions, we’re removing barriers and creating lasting pathways to better health.” Through initiatives like Neighborly, Geisinger connects patients and members to more than 11,000 community organizations offering support with food, housing and transportation—supported by routine social needs screening and data driven insights. 📊 Impact across our communities includes: • Supporting 75,000+ individuals with food access and providing 3.5 million pounds of food • Connecting 1,500+ people to housing and utility assistance • Coordinating 43,000+ rides to health and social care appointments • Enrolling 150+ individuals in GED education support These efforts are powered by Geisinger’s Social Needs Resource Hub, launched in August 2025, which bridges health care and social services to reduce disparities and improve outcomes. We’re honored to be recognized—and even more proud of the difference this work makes in the communities we serve. Read more: https://lnkd.in/gMDyGnzJ
