Ten years ago, I was the Inspector General of a fighter wing, running self-inspections meant to keep units honest before small problems became big ones. One inspection — of the only operational flying squadron on the base — turned up real leadership issues, and the reaction from above taught me more about institutional accountability than any of the twenty-five inspections before it: independence was a façade, and the “watchdog” system we worked in really had no teeth.
