Malaysia Needs to Rethink Ex Regulation — Before the Next Incident Just this week alone, we’ve seen three separate fire and explosion incidents — the refinery explosion and fire in Geelong, Australia; the steel factory explosion in Gebeng, Kuantan; and the fire that destroyed more than 1,000 stilt homes in Sandakan, Sabah. In 2026 alone, more than a dozen explosions have been reported worldwide — from Kuala Lumpur to Utrecht, New Jersey to Andhra Pradesh and Maharashtra. These are only the reported ones. More than 180 fatalities have already been recorded from major incidents this year alone. The pattern is clear. Fatalities are clustered in preventable environments, with recurring causes: ❌ Poor maintenance of degraded equipment ❌ Improper handling of flammable materials ❌ Weak compliance ❌ Workflow and human failures These are not black swan events. Explosions are not rare — they are systemic failures that repeat. And they don’t start with explosions. They start with things we choose to ignore — a loose gland, a degraded cable, a shortcut during installation, a box ticked instead of a condition verified. Small things. Until they’re not. Malaysia needs to rethink how we regulate Ex — before the next incident happens. 1️⃣ Make Ex compliance mandatory — not implied Extend beyond O&G into chemical, pharma, food processing, agriculture, semiconductor — any industry exposed to gas and dust explosion risks. 2️⃣ Enforce competency — not just certification Stop accepting paper qualifications. Require demonstrated, auditable competency — for both personnel and service providers. 3️⃣ Institutionalise inspection & maintenance Periodic Ex inspection must be regulated and enforced — not left to operator discretion. 4️⃣ Digitise traceability Every Ex asset should have a lifecycle record — installation, inspection, modification. If it’s not traceable, it’s not compliant. 5️⃣ Align incentives — not just penalties Today’s system rewards ticking KPIs and closing jobs. We need to reward actual integrity outcomes, not paperwork. The industry knows the risks. The standards already exist. What Malaysia needs now is alignment — across regulators, operators, and service providers — to make Ex integrity a national priority, not a discretionary practice.
