Clinical Case Snapshot | Chronic Periapical Pathology A patient presented with a chief complaint of pain in the lower left posterior tooth region. The pain was mild, continuous, non-radiating, aggravated on mastication, and relieved by medication. Clinical examination revealed a grossly decayed tooth irt #36. Radiographic evaluation using RVG revealed a well-defined periapical radiolucency associated with the mesial root of a mandibular molar, suggestive of a periapical granuloma with associated external root resorption. The distal root demonstrated periapical sclerosis, indicative of a reactive osseous response to chronic low-grade inflammation. This case highlights the varied radiographic manifestations of chronic periapical disease, where both resorptive and sclerotic changes may coexist around different roots of the same tooth.
