Super Microsurgery in plastic surgery can also be a powerful tool when collaborating with other specialities and not just for free tissue transplantation. Superficial temporal artery bypass to Middle cerebral artery is an operation I perform routinely with my colleague Mary Murphy for cerebral revascularisation. The STA can be dissected under the microscope and limiting thermal damage with use of micro ligaclips. The anastomoses are often 0.6 mm or less and easiest performed with a posterior wall technique end to end and in some cases end to side if great mismatch. 10.0 or 11.0 sutures  often needed for vessel repair. At the end indocyanine green is used to check flow. With a neuro-plastics approach 2 cases can be accomplished in a day ! To date we have done over 36 cases.
