Washington families and case managers — here’s a pattern I keep seeing: Care doesn’t break because of one big mistake. It breaks because no one is tracking the small ones. Late arrivals. Missed steps. No clear updates. Individually? Minor. Together? They change everything. One case manager said: “I only hear about issues when families complain.” That’s already too late. Simple fix that works: For families: • Ask for 3 bullet updates after every visit • Same format. Every day. For case managers: • Track patterns, not incidents • Look for consistency across caregivers Real example: Family felt “something was off.” No clear issue.
