Happy International Tourette Awareness Day! Tourette Syndrome is a neurodevelopmental disorder, characterized by motor- and vocal tics.   A tic is defined as a sudden, involuntary, rapid, periodic, and purposeless movement (motor tic) or vocalization (vocal tic).    The exact pathophysiological mechanisms of Tourette Syndrome are not fully understood but several key findings and hypothesis exist from dopaminergic dysregulation, reduced GABAergic inhibition to neurinflammatory and immune mechanisms or genetic or epigenetic factors.    Did you know that also in Tourette syndrome the Endocannabinoidsystem is dysregulated? Müller-Vahl and working group have found that in patients with Tourette, the most important endogenous endocannabinoid messengers, namely AEA, 2-AG, PEA and AA were significantly increased in comparison to controls. Read more: https://lnkd.in/dk5_ZgYR    Furthermore, preclinical research has found that CB2 receptors play a role in the development of motor tics in mice. Read more: https://lnkd.in/dXBdXAnC    While real-world data and open-label studies suggest that cannabinoids may alleviate tics in individuals with Tourette syndrome, the current evidence from randomized controlled trials remains limited and inconclusive. Translating these observational benefits into statistically significant findings in RCTs has proven challenging. Recent systematic reviews and expert consensus still indicate that cannabinoids, alongside dopamine D1 receptor antagonists such as ecopipam, represent among the most promising investigational treatment options for patients with treatment-refractory tics.
