Catheters In, Image Out. Recently we did a T-TEER with the TriClip at Maastricht UMC+. The initial 3D TEE was beautiful. Crisp leaflets, clean MPR, exactly what you want before you start. Then the catheters went into the right atrium.... And the image fell apart. We could not tell whether the leaflets were sitting on the clip arms. Not on 3D MPR. Not on 2D biplane. Not even on a deep transgastric short axis. Based on TEE alone, this procedure was not doable. With ICE everything came back. Clear leaflet visualization, confident grasping, real time confirmation. After the first clip my genuine reaction was "thank god we've got ICE", because I regained confidence this was possible. We placed three clips and went from massive TR to trace TR. 3D ICE is not only a backup tool anymore.
