Shoutout to our Aberdeen Discharge to Assess (D2A) team in Scotland Within the first 4 months of launching in Aberdeen City, the service has already saved 736 bed days, demonstrating a meaningful impact early on. Working in partnership with Aberdeen City Health and Social Care Partnership, NHS Grampian, and the wider social care system, the team is helping people leave hospital sooner and return home with the right support in place. The achievements so far are incredible: 🟢736 bed days saved (within the first 4 months) 🟢 People supported to transition safely from hospital to home 🟢 Individuals enabled to recover at home with tailored support that tapers as independence grows 🟢 Increased capacity in acute settings for those who need it most Due to its success, the service has expanded significantly - but it’s the human impact that truly stands out: “I am writing to express my sincere gratitude for the exceptional care and support you provided to Paul (name changed) during his time with the Red Cross. Your dedication and compassion made a significant difference in his experience. Paul spoke highly of the kindness and attention he received from your team. Your commitment not only to his physical needs but also to his emotional wellbeing did not go unnoticed. It was comforting for us to know that he was in such capable hands during a challenging time.” “I wanted to express our gratitude once again for the care mum has received from your entire team. Without the care and support mum would not be in the healthy position that she now finds herself.” A huge well done to the Aberdeen D2A team for delivering such essential, community-based support.
