Research puts the intraoperative documentation error rate at 17% under normal OR conditions. When documentation is delayed more than 20 minutes, that rate climbs to 38.4%. Circulating nurses track every supply, implant, and tissue used, all while managing patient safety, sterility, and room turnover. When those priorities compete, documentation waits. Items get logged after the patient leaves, reconstructed from packaging and memory. That reconstruction is what feeds charge capture, EHRs, and every analytics platform built on top of them.
