We’re proud to share our 2025 Impact Report, highlighting how agilon health continues to empower physicians, advance value-based care and strengthen the communities we serve. As healthcare becomes more complex, especially for seniors managing chronic disease, agilon continues to invest in clinical pathways, care coordination and data-driven insights that help identify risk earlier, connect patients to the right care sooner and help keep them out of the hospital. From heart failure to other chronic conditions, we are continuing to build and scale programs that support more proactive, connected care across the communities we serve. In 2025, more than 600,000 seniors were cared for through a total-care relationship with an agilon physician partner across 30 communities nationwide. Compared to Medicare fee-for-service benchmarks, agilon patients experienced: · 8% lower ER utilization · 10% lower hospital admissions · 24% lower readmission rates We’re grateful to our physician partners and teams across agilon who make this work possible every day.
