The most expensive study is often the one that answers the wrong question. In Medical Affairs, we spend a lot of time discussing study design, endpoints, and methodologies. But one question should come before all of them: What decision are we trying to influence? Whether it’s payer review, clinical adoption, MLR approval, or market access planning, the decision should guide the evidence plan, not the other way around. When the decision comes first, evidence generation becomes more focused, evidence gaps become clearer, and investments are aligned to impact rather than activity.
