For Jerred, a nurse in our Hospital at Home program at Advocate Lutheran General Hospital, every day is about delivering high-quality care where patients feel most comfortable, in their homes. His day starts with a 7 a.m. team call to align patient needs, followed by coordinating with providers and heading out with supplies to begin in-home visits. Patients receive two nursing visits daily, where Jerred provides hospital-level care, from IV medications and lab draws to wound care, assessments and virtual visits with hospitalists and specialists. A key part of his role is patient education, helping individuals manage medications and better understand their conditions, with support from a collaborative virtual care team. One experience stands out, a patient in her early 60s with heart failure who spent nearly three weeks in the program. After transitioning from the hospital, she was able to recover at home, surrounded by loved ones. “She was less anxious, more rested, and told us she would have gone crazy staying in the hospital that long,” Jerred shared.
