✨ Latest Update Now Live: Reflecting on Frailty‑Attuned Care Across the North West ✨ We're delighted to share the third update in our ongoing reflective blog series showcasing Aqua’s work with NHS England North West as we deliver three major Improvement Collaboratives across urgent & emergency care, elective care and adult mental health. This latest chapter captures the highlights from Learning Event 3, where we were joined by Dr Tom Downes, National Clinical Frailty Lead for NHS England, for an inspiring and deeply thought‑provoking session on frailty‑attuned care. Across the day, colleagues from acute, community, social care and the voluntary sector came together to explore: ⭐ What truly frailty‑attuned systems look like ⭐ The four key enablers of change – from collaborative leadership to system‑aligned finance ⭐ The evidence behind Hospital at Home and neighbourhood‑level discharge How we can build shared culture, capability and confidence to drive meaningful transformation ⭐ A recurring message throughout the day was the simple but powerful principle: “Fix frailty, fix the NHS.” It was energising to see so many teams reflecting honestly on their strengths, challenges, and opportunities, and identifying practical next steps for their local areas. Momentum is now building as we head toward Learning Event 4, where teams will begin sharing impact and celebrating progress from the past year. We’ve also got further learning events and an action‑learning approach on the horizon, supporting teams to explore real‑world methods for reducing anticholinergic burden.
