When Risk Is Ignored, This Happens. In this accompanying video there was an Attempt to lift a bundle of beams using three forklifts and it went terribly wrong, bundle shifts, forklifts overturn, carts are scattered. This didn’t have to happen. This video is a stark reminder of what goes wrong when we fail to assess risks properly and ignore equipment limitations. 1. No clear lifting plan 2. No load stability check 3. No consideration of Safe Working Load (SWL) 4. No checks for leveled ground Surface 5. No proper coordination among the operators The result? 1. Forklifts overturned 2. Load scattered 3. Lives and equipment unnecessarily put at risk Risk Assessment is not a box to tick. It’s a proactive step to ask: 1. What could go wrong? 2. Are we using the right equipment? 3. Is this equipment within its rated capacity? 4. Do we understand the dynamics of the task ahead? When multiple forklifts are used to lift a single load, it introduces complex weight distribution risks, especially if they're not perfectly synchronized or if the ground is uneven. One wrong move and the entire system collapses, just like in the video. Let’s make this clear: 1. Forklifts are not cranes. 2. SWLs exist for a reason. 3. Guesswork has no place in safety. Before you lift, shift, or move anything: ✔️ Conduct a proper risk assessment ✔️ Use the right equipment for the task ✔️ Stay within the safe working load ✔️ Train and coordinate your team One moment of oversight can lead to months of damage, physical, emotional, and financial. Let’s do better. Let’s plan better. Let’s work safer. #HSE #RiskAssessment #SafeWorkingLoad #MaterialHandling #WorkplaceSafety #ForkliftSafety #ZeroHarm #EquipmentSafety #SafetyLeadership #IncidentPrevention #ThinkSafeWorkSafe #ConstellationHSE #LessonsLearned #HazardAwareness #OperationalExcellence
