Patient with chronic headaches and right-sided tinnitus referred from ENT. Angiography: Enlarged emissary vein and occipital plexus associated with internal jugular stenosis with secondary enlargement of IJV. Treatment: transradial combined with transvenous approach. Couls and NBCA of emissary vein and stenting of IJV. 6-mo fu: patient asymptomatic, no tinnitus. Confirmed permancy of occlusion and patency of IVJ with secondary size reduction. Termination of Brilinata. Take home message: the venous system needs more attention and is still an exciting and uncharted discipline in NIR.
