You cannot plan the reconstruction once the tumor is removed. You must plan it before. When an intraosseous meningioma invades the temporal bone and distorts the orbit, three challenges must be addressed in a single preoperative planning session. The first was the resection. A patient-specific cutting guide was designed to remove the affected temporal bone along a planned trajectory, providing controlled surgical access to the orbit. The second was orbital reconstruction. The contralateral orbit was mirrored digitally and used as the geometric blueprint for a custom implant, shaped on a 3D model during surgery to restore the original orbital volume. The third was the cranial defect. A patient-specific mold, prepared in advance, allowed the cranial implant to be formed and placed within the same operative session.
