TOP PUBLICATION Novel challenging memory tests for early Alzheimer’s disease show promise, but clinical translation remains limited 📄 Our new open-access paper in Neuroscience and Biobehavioral Reviews As Alzheimer’s disease diagnosis moves into earlier biomarker-defined stages, we are increasingly trying to detect changes before clear clinical symptoms emerge. A new generation of more demanding (“challenging”) memory tests aims to capture very subtle, preclinical changes that standard tests may miss. Some results are encouraging. There are associations with biomarkers, early group differences, and even some predictive signals. However, an important limitation remains: 👉 None of these approaches is currently ready for routine clinical use. What is missing is not sensitivity, but translation: norms, cut-offs, and reliable interpretation at the level of an individual patient. For now, these methods function more as markers of early cognitive vulnerability than as clinical diagnostic tools. We also discuss how, in a biomarker-driven era, the requirements for validating cognitive tests are shifting, particularly with respect to their use in preclinical, individual-level decision-making.
