💼 Position: Nurse Case Manager II (RN)
📅 Duration: 6+ Month Contract (Potential Extension)
🕒 Schedule: Monday – Friday | 8:00 AM – 5:00 PM
🏠 Telephonic & Field-Based Role
💰 Mileage Reimbursement Provided
Pay Rate - $45.00/Hourly - W2
Requirements:
✅ Active and unrestricted Virginia RN License or Compact RN License
✅ 3+ years of clinical nursing experience (Hospital, Home Health, Ambulatory Care, etc.)
✅ Case Management experience preferred
✅ Managed Care experience preferred
✅ Strong communication and computer skills
✅ Dedicated home office/workspace for telephonic case management
Key Responsibilities:
• Conduct comprehensive member assessments
• Develop and monitor care plans
• Coordinate care with providers and community resources
• Perform telephonic outreach and field visits
• Support members with health education and care coordination
• Document all activities in the case management platform
