I am an Occupational Therapist who has developed a myriad of theoretical and practical knowledge over the last 3 years, supported with knowledge and skills gained from an undergraduate in science. Engaging in volunteering, employment and clinical placements in various settings (hospital, aged care, community) has facilitated the development of skills and attributes important to Occupational Therapy practice, such as communication, teamwork, problem-solving and time management.
- Conduct comprehensive assessments to evaluate a participant’s physical, cognitive, sensory, and psychosocial functioning. - Identify barriers to daily living tasks such as self-care, mobility, communication, social participation, and community access. - Collaborate with participants, families, and support coordinators to develop person-centred therapy goals aligned with the NDIS plan. - Address short- and long-term goals using evidence-based practice and strengths-based approaches. - Implement strategies to increase independence in activities of daily living (ADLs), including personal care, domestic tasks, and social/community participation. - Develop routines, adaptive strategies, and skill-building programs for home, school, or work environments. - Assess, recommend, and trial assistive technology (e.g. lift recliner chairs, wheelchairs). - Prepare detailed clinical reports and recommendations to support NDIS plan reviews, funding applications, and service justifications. - Maintain accurate and timely documentation in line with NDIS quality and safeguarding requirements. - Work closely with other allied health professionals, support workers, educators, and medical practitioners. - Contribute to coordinated care planning, especially for participants with complex or high needs. - Provided clinical supervision and mentorship to Occupational Therapy students during fieldwork placements. Responsibilities included developing learning objectives, facilitating skill development, offering regular performance feedback, and evaluating student progress in accordance with academic and professional standards. Fostered a supportive and collaborative learning environment to enhance students’ clinical reasoning, therapeutic skills, and professional behaviours in real-world settings.
Rosebud Hospital - Subacute inpatient rehabilitation/ GEM This role involves working primarily with a GEM caseload together with the multidisciplinary team to ensure safe and appropriate discharge of patients, including complex discharge planning for patients. Common occupational performance issues included difficulty completing personal activities of daily living (PADL) and domestic activities of daily living (DADL) due to changes to a patient's mobility and strength. Assessments: - MoCA/MMSE/OT Cognitive screen - Functional Assessments (PADL/LMP) - Initial assessments - Home visits Interventions: - Pressure Area Care and education - Education for patient and carers on falls prevention and energy conservation strategies - Referral to post-acute services and MAC - Equipment prescription (e.g., pressure care equipment, shower chair, long handled aids etc.) - Education/liaise with family members regarding level of care patient would need
