To make a good use of my knowledge, experience, and charismatic nature as a pharmacist to improve the efficiency, safety, and progressiveness of a healthcare organization. My Objective is to make sure patients are given the best regimen possible during their stay. It is also my goal to make sure their home medications are not only the same while inpatient but that their doses are also correct based on labs, age, weight, etc. that way the patient goes home on an optimal regimen. I want to be able to be a resource for doctors and nurses; to be able to provide information on medications and give alternatives if something is not an option.
- Review, verify and dispense medication orders. - Prepare IV admixtures using aseptic technique & compound bulk medication preparations. - Perform retrospective & prospective Drug Utilization Review including monitoring of drug-drug interactions, drug-food interactions, and potential adverse reactions - Handle clinical staff questions regarding PO to IV conversion, formulary alternatives, IV compatibility and renal/hepatic dosage adjustment. - Perform nursing unit and in-house inspection to assure proper maintenance and storage of medication supplies as well as code cart medication supply. - Familiar with the operation of Pyxis automated dispensing machines & Epic. verifying medication orders, entering chemotherapy, making IV admixtures, checking medications, narcotic pyxis sends, compounding oral syringes, answering doctor/nurses questions, calling outpatient pharmacies/confirming home medications, identifying home medications for inpatient use Experience in: Cerner, Simplifi 797
-Provide patient-specific counseling on prescription & over-the-counter medications. -Initiate Medication Therapy Management (MTM) sessions which include prospective and retrospective Drug Utilization Review (DUR). -Manage pharmacy workflow including delegation of various job tasks among pharmacy staff. -Administer vaccinations and prepare compounded medications. Verifying medication orders, performing sterile IV prep including adult and neonatal TPNs, pediatric preps, chemo, education of health professionals about various medications as well as interacting with doctors about appropriateness of patients current medications, appropriate antibiotic use pre and post surgical (SCIP guidelines), counseling of patients on medication purpose/side effects, monitoring renally dosed medications, filling outpatient prescriptions, performing medication reconciliations on patients to have the most up to date list of home medications.
