Providing support to people with psychosocial disabilities to increase their independence and social & economic participation. Coaching & collaborating with other services to build capacity and to plan & implement a recovery plan.
• Developing recovery-enabling relationships with people experiencing a psychosocial disability or other complex mental health issues. • Developing recovery planning documents and supporting people with their recovery planning. • Coaching people to build personal capacity, motivation, resilience & decision making. • Collaborating with broader system of supports to aid in a recovery-oriented approach. • Supporting engagement with NDIS, including Plan implementation. • Documenting & reporting progress, and developing systems to do so from scratch as a pioneer in this position at Pillar. • Coordinating the Youth Psychosocial Support Packages (YPSP) program, funded and overseen by the Mental Health Commission (MHC). • Processing YPSP applications & liaising with clinical referrers, young applicants, and parents to assist in navigating through the process. • Liaising with external stakeholders & MHC to process applications and ultimately issue packages for support; reporting feedback & application statistics to MHC. • Delivering information seminars in person & online to external stakeholders & potential referrers.
Managing client relationships, budgets, funding utilisation and staffing for portfolio of clients and their support networks. • Liaising with client teams including support coordinators, recovery coaches, therapy providers, government representatives to collaborate towards client outcomes. • Completing NDIS plan reviews and service agreements for clients. • Providing leadership, coaching and support to team of line managed support workers (mentors). • End-to-end recruitment of mentors; from shortlisting and contacting applicants to induction and training. • Monitoring and approving staff timesheets and case notes. • Delivering (and attending) workshops for professional development. • Collaboration with coordinators and management team to continually reflect upon business processes and initiatives.
