Proven record in delivering optimal client outcomes by identifying barriers to care, creating care plans based on client's needs and medical history, assisting client in removing obstacles and navigating the healthcare system. Please note that I hold active RN licenses in PA and VA.
• Provided mental health crisis intervention, case management, intensive outreach and engagement services, education, skill building, problem solving training, and socialization coaching for severely-mentally ill clients in the community
