Private duty Nursing to one patient in family home. All needs attended to.
I gather information from the patient regarding their health concerns when they call into the clinic and then forward that to the Physician. The Physician responds with their orders and we let the Patient know what those are and then follow up in a couple of days. I also see patients who just show up at the clinic if they do not have an appointment with their Physician and there is no room on a Physicians's schedule. I triage them and then the Provider determines if they need to see them or if they can just give verbal orders. I also assist the Medical Assistants with their clinical skills when needed. Performing clinical skills within the LPN scope of practice.
Creating a plan of care for the patients whose PCP's have opted into the Silver State ACO. Following patients' needs and making sure they are transitioning from the Hospital home with everything they need so that they don't end back up in the Hospital.
