I am currently working in a locum role as an Experienced Social Worker, in the Forest of Dean, Adults Community Team; supporting individuals over the age of 18 to access Adult Social Care and Support Services. The team promote independence to enhance and empower individuals in leading their best lives possible. This role includes conducting care act assessment and support planning. Legal assessments, concerning decisioning making, mental capacity and best interest decision making, front door duty and safeguarding enquiries alongside collaborative work with multi-discplinary colleague's and wider community services.. I enjoyed my leadership roles but have now expanded my expertise within mental health, learning disabilities and autism alongside to include Adult Social Care. I am also open to strategic/or enabling project work with specific client groups in frontline roles in locality and multi-disciplinary teams. Previously, I was leading a small hub of workers within the Gloucestershire locality, comprising of Social Workers, Social Work Apprentices and Social Care Assessors. I succeeded my role of Senior Social Worker to become a lead within the locality of which I have been in position for over 2 years now and thoroughly enjoy this role. Leadership is a vital and important role in the social work field, especially in such unprecedented times as recently. My strengths lie in communication, peer and group mentoring, working well under pressure and providing excellent resources, practice guidance, praising success and identifying performance issues and development needs. I strongly advocate social work values and promote autonomy amongst my supervisees to develop their own professional styles. I enjoy strategizing personal care plans for individuals that may present to services, that experience disadvantage and may often fall between services, in order to reach better outcomes and reduce the need for revolving services in their lives. I enjoy improving pathways for individuals in the community to access quality support and and care and being creative with how services can adapt to accommodate new ways of working. I am passionate about the people I serve and work with and this is reflected in my personality and ability to perform.
Front Door Hub Duty Responsibilities Conduct Care Act (2014) S.9 Assessments, Support Planning in line with make the difference, 3 conversation model of practice. Conduct mental capacity assessments and prepare Court of Protection paperwork. Conduct Care Act (2014) S.42 Safeguarding Enquiries Work collaberatively within a multidisciplinary framework with complex case management Work alongside police, health and local authority partners to achieve support and care for hard to engage individuals. Working with client groups: Learning disabilities, Learning Difficulties, Substance Misuse/Dependency, Hoarding, Self Neglect and Domestic Violence.
Completing assessments and reviews under the Conversation / Strengths Based Practice Model. Undertaking multidisciplinary assessments with the individual / care team to ensure appropriate health pathways are identified. Working alongside young adults pre/post 18 with autism, learning disability or care and support needs who was a looked after child or in receipt of Special Educational Needs and Disability (SEND) and Educational Health and Care Plans (EHCP). Promoting greater understanding between differing eligibility criteria for public funds between adult and children's services. Undertaking capacity assessments for court of protection applications (COP3), those that are of complex nature surrounding sex and online contact, community deprivation of liberty safeguards. Budget applications, provider disputes and troubleshooting identified issues to effect collaborative working practices and continuation of care. Safeguarding enquiries and concerns completed.
* Line manage a team of social workers and social care practitioners, including supervision, absence, appraisal and performance management. * Provide individual/group professional, reflective supervision, and support to others, ensuring that practice is continuously critically reflected upon and improved; modelling best practice and ensuring good outcomes. * Sign off temporary funding or service provision (within a budget limitation) * Responsible for completing case audits ensuring that social work practice is continuously improved and compliant with national guidance, legislation, LA polices and procedures. * Work jointly with staff and agencies on highly complex cases, working closely with the senior social worker and wider staffing group to coordinate and manage section 42 enquiries and complex, high risk situations, ensuring there is a positive balance between perceived risk and protection from harm where necessary. * Attend or chair relevant meetings (including risk management multidisciplinary) which require high level professional input or where I may need to support the social worker, including going to Court. * Support with complaint management procedures. * Support with recruitment and retention activities, including interviews, shortlisting, induction and probations. * Create a culture of continuous professional development. * Currently undertaking mentoring of new Student Social Work Apprentices. * Complex Case Work with mental health, continuing health care, multi-disciplinary funding implications. * I promote positive working relationships in and across teams, using strategies for collaboration and contribute to a supportive organisational culture.
My role was to provide expert and effective Social Work practice guidance in case management, assess and carry greater responsibility for individuals with higher levels of risk and management of competing demands within a complex caseload. The senior role offers expertise in specialism through historical case management and effective working practices. Underpinning human rights, social work values and striking balances between support and control within individuals lives. This would require leadership in liaising with a wide range of professionals, including more senior levels and case of higher profile. This would include co-ordinating and administering multi-disciplinary pathways and chair meetings to include safeguarding, best interest decision meeting, case reviews and conflict management. Alongside a complex caseloads my duties were to include advice, support and enhancing development in others practice, forging collaborative networks and contacts within partner organisations, whilst underpinning quality in practice and assessment. I would create and develop training sessions and mentoring for students when undertaking Assessed and Supported Year in practice, shadowing opportunities and sharing knowledge through participation in expert testimony and practice . I comprised a number of learning tools and resources for students and progressing social care practitioners to enhance their practice also.
